Developing and Following Your Plan for Care
Program Description: If something happened to you, and you were no longer able to care for your patients, would the person stepping in to fill your role pick up your care where you left off? That’s why care planning is an essential part of home health, though many times it’s misunderstood or regarded as a waste of time. Without a specific document outlining the plan of care, important issues are likely to be neglected.
Our goal today is to provide guidance so you’re able understand the key elements of a care plan and fill one out properly. Remember, a caregiver unfamiliar with your patient should be able to find all the information needed in the care plan.
Objectives:
- Identify criteria for admission to home care services.
- Know the components in the Plan of Care (POC).
- Understand listing and changing medications and supplies.
- List the four appropriate Functional Limitations and Activities Permitted.
- Recognize and implement appropriate goals, rehabilitation potential and discharge plans.
