One of the chief goals of health care reform is to improve quality and cost effectiveness by reducing fragmentation across providers and systems. Improving provider communication, coordination and patient care continuity will help to ensure that patients receive high-quality care and prevent unnecessary hospital readmissions. This webcast will focus on why cross-setting collaboration across all healthcare provider and community service settings is important for achieving improved outcomes. Webcast speakers will review the driving forces for improved transitional care management and patient/caregiver activation at the local, state and national levels and discuss strategies to include caregivers in the care transition team. Examples of organizational culture change and patient/caregiver activation will also be discussed, with an emphasis on improving patient outcomes through development of strong partnerships across the healthcare continuum and expansion of caregiver roles that promote successful community living.
